Default Order Forms help wanted?

I was wandering if anyone could help me out with some kind of order form for crosses and portraits and such. I am not good at typing that stuff up. All help is greatly Appreciated. I am getting all stuff covered for my 1st show on the 14th of November so if you all have any other suggestions Please let me know. This message board is the best thing since sliced bread....

Years ago I bought a pad of forms at Home Depot. They were proposal forms for contractors. They had a lot of other forms too. Worth a looksee....

You can get different style of receipt books at Walmart, Staples ect... that work well as order forms. I had mine printed from an online company that was rather cheap. I got the carbonless forms. Can not remember the companies name. Steve

I bought a pad at the dollar storefor receipts and I use a nice book also from the dollar store to keep track of special lettering orders
