Musical Notes patterns - Scroll Saw Woodworking & Crafts Message Board

I lost my copy of Scrollsaw that had a pattern in it for a musical stanza. It included a melody and showed how to change the notes. I cannot find it anywhere. My wife is a wonderful piano and organ player and I want to make it for her for Xmas. Help!!
